Chichester District Older People’s Partnership works closely with the Chichester Community Partnership Team (CPT).

The CPT is a joint team including staff from West Sussex County Council, West Sussex Primary Care Trust, Department for Work and Pensions, Age Concern West Sussex, the Alzheimer’s Society and Mind Western CIC.

The CPT will work with an older person to advise or actively put in place a range of measures which will assist that person’s health, quality of life and ability to live independently.
